The short version — the current evidence does not show that creatine causes hair loss. What the 2025 study actually found
In 2025, researchers ran the study that had been missing all along. Forty-five resistance-trained men aged 18–40 were given either 5g of creatine monohydrate a day or a matching placebo for 12 weeks. This time they measured the thing everyone actually cared about: hair.
They tracked testosterone, free testosterone, DHT, and hair follicle health — density, follicle count and hair thickness — before and after. The result: no significant difference in DHT, in the DHT-to-testosterone ratio, or in any hair measure between the creatine group and the placebo group. The authors described it as the first study to directly assess hair follicle health after creatine supplementation, providing strong evidence against the claim that creatine contributes to hair loss.
It lines up with earlier reviews too. A widely cited 2021 review of the common creatine myths concluded that the evidence does not indicate creatine raises total testosterone, free testosterone, DHT, or causes hair loss.
Does creatine raise DHT?
This is the real crux, because DHT — not creatine — is the hormone with an actual link to male-pattern baldness. The honest answer: the 2009 rugby study is the only one that ever suggested a meaningful DHT rise, and it has never been repeated. The 2025 trial specifically measured DHT over 12 weeks and found no significant change versus placebo.
In other words, the single domino the whole myth depends on — creatine reliably raising DHT — doesn't hold up when it's actually tested.
What actually drives hair loss
If you are thinning, the usual culprits are far more boring than a tub of powder.
